Adult Palatal Expansion Without Jaw Surgery
A narrow upper jaw can crowd your teeth, create a crossbite and leave less room for air to move through the nose. In growing children, a standard palatal expander can usually widen the jaw before the bones finish developing. By the late teens and adulthood, the seam that runs down the roof of the mouth has typically fused, and a standard expander tends to tip the teeth outward instead of widening the bone. At Mia Orthodontics, board-certified orthodontist Dr. Alireza Hourfar uses MARPE (miniscrew-assisted rapid palatal expansion) to widen the upper jaw in older teens and adults, often as an alternative to jaw surgery.
What Is MARPE?
MARPE is a palatal expander anchored directly to the bone of the palate with small titanium miniscrews, also known as temporary anchorage devices (TADs). Because the appliance pushes against bone rather than relying only on the teeth, it can apply steady, controlled pressure to the two halves of the upper jaw and gradually separate them at the midline. It uses the same anchors we rely on for mini-implant assisted orthodontics, and it is one of several forms of specialized orthodontic care we provide for complex cases.
In some cases, Dr. Hourfar pairs MARPE with piezo-assisted corticotomies, a variation called MARPPE, to enhance the bone’s response and improve comfort during expansion. Whether that step makes sense depends on your age, your bone and the amount of expansion you need, all of which we evaluate before treatment begins.
Who Is a Good Candidate for MARPE?
MARPE is designed for patients whose upper jaw is too narrow for the lower jaw or for the teeth it needs to hold. You may be a candidate if you:
- Were told you might need jaw surgery to correct a narrow upper jaw
- Have a crossbite, where the upper back teeth sit inside the lower teeth
- Have crowding or not enough space for your teeth to line up
- Have breathing or airway concerns related to a narrow palate
- Are an older teen or adult whose growth plates have already fused
Younger children usually do well with a conventional expander or, for some patients, the Invisalign Palatal Expander. MARPE is reserved for patients whose bone has matured past that stage. The miniscrews need healthy gums and bone to hold securely, so a full exam and 3D imaging come before any recommendation.
Benefits of MARPE
- Skeletal change, not just tooth tipping: MARPE widens the upper jaw itself, which creates more stable space than moving the teeth outward alone.
- A non-surgical option for many adults: Patients who were told surgery was their only path to a wider jaw can often be treated in the office instead.
- Room for alignment: Added width gives crowded teeth space to move into position.
- Better bite alignment: Widening the upper arch helps correct crossbites so the upper and lower teeth meet properly.
- Airway support: When a narrow palate contributes to breathing problems, expansion can help open space for airflow.
- Works with your next phase: MARPE is followed by braces or clear aligners to finish the bite.
MARPE vs. Traditional Expanders vs. Surgical Expansion
| Traditional Palatal Expander | MARPE | Surgically Assisted Expansion | |
|---|---|---|---|
| Typical patient | Growing children | Older teens and adults | Adults whose needs go beyond what MARPE can achieve |
| Anchored to | The teeth | Miniscrews in the palate bone | Bone released during surgery |
| Procedure | No surgery | In-office miniscrew placement with local anesthesia | Surgery performed by an oral and maxillofacial surgeon |
| Main limitation | Less effective once the palate has fused | Depends on bone maturity and density | Surgical recovery time |
When surgery is the right answer, Dr. Hourfar works with experienced oral and maxillofacial surgeons to coordinate corrective jaw surgery alongside your orthodontic care. For many patients, though, MARPE makes surgery unnecessary.
The MARPE Treatment Process
- Consultation and 3D imaging: We examine your bite, jaw width and airway, then use our digital 3D X-ray system, which includes CBCT, to measure the palate and the bone that will hold the miniscrews.
- Digital treatment planning: Dr. Hourfar maps miniscrew positions and designs an expander that fits your palate.
- Placement: The miniscrews and expander are placed in our office with local anesthesia. Most patients feel pressure rather than pain.
- Expansion: The expander is activated in small increments on a set schedule. As the upper jaw widens, a temporary gap often opens between the front teeth, which is a sign the bone is separating as planned.
- Stabilization: The appliance stays in place for several months while new bone fills in and the expansion settles.
- Alignment and retention: Braces for adults or clear aligners close the gap and refine the bite, and a retainer protects the result.
Why Choose Mia Orthodontics for MARPE?
Skeletal expansion in adults is not offered at every orthodontic office. Dr. Hourfar’s practice focuses on complex cases, from skeletal open bites and gummy smiles to molar intrusion and adult palatal expansion, using TADs as a non-surgical alternative to jaw surgery whenever that is a realistic option. Our fully digital workflow, from 3D scanning to treatment simulation, lets us plan expansion precisely and show you what to expect before anything is placed.
When a narrow palate is tied to breathing or sleep concerns, we connect MARPE with our airway orthodontics approach and coordinate with sleep physicians and ENT specialists for proper diagnosis. Our office at 3636 Greenbriar Dr., Ste A-101, near Upper Kirby, offers flexible scheduling and ample parking, and our team will walk you through insurance and payment options before treatment starts.
MARPE FAQs
Does MARPE hurt?
The miniscrews are placed with local anesthesia, and most patients describe the procedure as pressure rather than pain. During expansion you may feel tightness in the palate, cheeks or the bridge of the nose for a short time after each activation. Over-the-counter pain relief is usually enough.
How long does MARPE treatment take?
The active expansion phase is usually short, often a matter of weeks. The appliance then stays in place for several months so the new bone can stabilize, followed by braces or aligners. Your total timeline depends on your treatment plan, which Dr. Hourfar will outline at your consultation.
Am I too old for MARPE?
There is no fixed age cutoff. Success depends more on bone maturity and density than on age alone, which is why we evaluate every adult patient with 3D imaging before recommending MARPE.
Can MARPE help with breathing or sleep?
Widening the upper jaw can increase space in the nasal airway for some patients. MARPE is not a stand-alone treatment for sleep apnea, so when breathing or sleep concerns are part of the picture, we work with sleep physicians and ENT specialists to make sure you get the right diagnosis.
Why is there a gap between my front teeth?
A gap between the upper front teeth is a normal, temporary part of expansion. It shows the two halves of the jaw are separating, and it closes during the alignment phase of treatment.
How much does MARPE cost?
Cost depends on your treatment plan, including how much expansion you need and what follows it. MARPE is usually part of a larger orthodontic plan, and insurance coverage varies. We review your benefits and flexible payment options with you before treatment begins.
